An Intake Sheet is a short one page summary of the claim you are submitting for collection. It is personalized to your office so we know right away which client submitted the claim. Answer the questions below: list the name of your bonding company, your address, the City/State/Zip where you do business, your telephone number and your fax number. Finally, enter your email address and within a minute an intake sheet will be delivered to your email and you will be ready to begin.
